BEFORE YOUR VISIT: Please complete the corresponding intake form and consent forms (see below) and bring them with you to your visit. You may be asked to complete supplementary forms at the clinic. Please arrive to the clinic 15 minutes prior to your visit.
Complete the forms as thoroughly as possible, as the questions were designed to gather as much relevant medical information as possible. Naturopathic medicine takes into account the whole person to formulate a comprehensive treatment plan tailored to your individual needs. The more information we can gather the better informed we all are.
If you have questions regarding the forms please let us know and we will go through them together in your appointment. If you have any other relevant medical forms (e.g. previous laboratory test results, X-rays etc.), please make sure to bring a copy with you.
Initial Appointment (Duration: 60 minutes) The first visit includes an in-depth assessment of your current health concerns, medical history and health goals. Your Naturopathic doctor will review the intake form with you, asking detailed questions relevant to your health concerns. We may complete a screening exam, and if necessary run a in-office tests and/or request for blood work.
Please bring with you: • New patient intake form & consent form • List of supplements, medications (prescription and non-prescription), including doses • A copy of your most recent blood test results or any other relevant diagnostic test results
Second Appointment (Duration: 30-45 minutes) During your second visit, your Naturopathic doctor will continue reviewing your concerns and discuss treatment options that are suitable and available to you. Together, we will determine the best way to approach your health concerns, making your treatment plan not only individualized to your needs, but also simple to follow for compliance.
Subsequent Appointments (Duration: 15-60 minutes) Follow-up appointments are essential to short and long term progress, so together we can re-assess, re-adjust and refine treatment protocols whenever necessary. The frequency and number of follow up visits is case-dependent and will be discussed at the appropriate time.
